1. The word, 'Locum' in these terms and conditions refers to any employee recruited by AHSR to work for an Employer.
2. The word, 'Employer' in these terms and conditions refers to any person or organisation that undertakes to recruit staff through AHSR.
3. AHSR attempts to keep up to date with the requirements for Dental Board registration for each state. Occasionally, however, changes take place without our knowledge. The Locum retains full responsibility for ensuring that they are registrable in the state and country in which they are seeking employment and for obtaining their registration in a timely manner.
4.AHSR endeavours to ensure Locums are provided with written confirmation and the details of their placement with the Employer. Where the Employer does not draft an employment contract AHSR may draft one and require it to be signed by both Locum and Employer. AHSR will not be liable for difficulties or damages arising from a contract so drafted.
5.The Locum is to provide, in good faith, any information AHSR may require to enable AHSR to find the Locum a suitable position. Although AHSR endeavours to screen Employers where possible and match Locums to them well, the decision to accept work rests solely with the Locum. AHSR will not be responsible for the Employers competence or character or the Locums suitability to the job.
6.AHSR accepts no responsibility for any cost, damage or loss to the Locum arising from any behaviour, action or failure by the Employer.
7. The Locum shall not accept any modification or extension of any contract without first contacting and proceeding through AHSR.
8.The Locum shall not accept any offer from an employer introduced by AHSR of future work of any type or referral to another Employer within 18 months of introduction or the most recent contract without
9.The Locum must provide the Employer with all information required to ensure correct payments are made. Such information may include provider numbers, superannuation fund details and tax file numbers.
10.The Locum shall hold professional indemnity insurance at all times and be registered with the relevant authorities.
11.The Locum shall attend to all patients in a manner consistent with best medical/dental practice or in keeping with the nature of available resources.
12.The Locum shall attend to all duties promptly and efficiently.
13.The Locum shall at all times, for legal and ethical reasons, maintain patient confidentiality. In keeping with standard practice and within the confines of law and ethics, the Employer or other relevant authorities may at times be privy to patient information.
14.The Locum shall not, within the confines of law and ethics, disclose any information pertaining to the affairs and practice of the Employer.
15.The Locum acknowledges that the Employer may be asked to provide an evaluation of the LocumÕs performance. Information so obtained will be used solely for the purposes of facilitating future placements and to ensure some measure of quality control.

17.Once a Locum has given verbal acceptance of a position this is regarded as a firm commitment.
18.All Locums wanting work in NSW must sign a declaration stating that they are not prohibited from working with children under the Child Protection (Prohibited Employment) Act NSW 1998. All locums wanting work in NSW must undergo a Working With Children Check to ensure they are not prohibited under the aforementioned Act and sign a consent form to enable us to arrange the checks. Should a fee be levied by the Department of Health to cover the costs of this check, the cost will be passed on to the locum. It is an offence in NSW for prohibited persons to apply for work as dentists. AHSR is unable to assist prohibited persons in securing work.
19.AHSR retains the right to refuse to provide or discontinue providing services to any Locum.
20.Although AHSR endeavours to provide correct and up to date information pertaining to job vacancies both on its website and in its records, errors sometimes occur and employers sometimes change their minds. AHSR requests that the Locum verify job and remuneration details with the employer prior to accepting a contract. AHSR will not be liable for any loss or damage that the Locum sustains by relying on information provided by AHSR.
21.By choosing to proceed through AHSR the Locum agrees to abide by AHSRs terms and conditions.
